Community Nursing Visits
Community Nursing is provided by our Registered and Enrolled Nurses. Most nursing care can be completed during a short visit, which is less than or equal to 30 minutes. The hourly rate applies after this time.
When appropriate our Community Nurses may delegate routine aspects of your clinical care to our Home Care Assistants, who have the necessary competence to meet your needs.
In some circumstances, short notice requests for nurses may incur a minimum fee of 4 hours at the relevant hourly rate. We will advise you if this is the case prior to confirming the requested service.
Care Management
Care Management is an essential feature of your Support at Home services. Together we will create your Care and Services Plan, personalised to best meet your needs. Services provided to you will be co-ordinated, monitored and reviewed in order to support any changes, or adjustments, that may be required.
Care Management is capped at 10% of your Support at Home quarterly budget.

Sleepover
A sleepover is a 12 hour shift that includes 4 hours of active service and an 8 hour sleep period. If the Home Care Assistant is required to attend to the client’s needs for greater than one hour during the sleep period then an additional disturbance fee of $116 per hour will be applied.
Accompanied Activities
This is when your Home Care Assistant accompanies you on an outing or to an appointment.

Associated Providers are third party providers we contract to provide care and support services to you. When using an Associated Provider, we charge you their price plus 10% for overhead costs.
Assistive Technology & Home Modifications (AT-HM) Scheme
- Assistive Technology: Administration costs are capped at 10% of the item/item bundle cost or up to $500 (whichever is lower).
- Home Modifications: Co-ordination costs are capped at 15% of the quoted cost or up to $1,500 (whichever is lower).
- Unspent Funds: For AT-HM, HCP Commonwealth unspent funds must be used, if available, before AT-HM funding tiers are accessed.
